Home Support Services provides in-home assistance and support for older adults of all races and ethnicities. Our services enable older adults to remain independently and safely in their own homes. One of our program goals is to delay or prevent premature nursing home or other residential placement.
The JASC screens and trains its homemaker staff to provide culturally sensitive, supervised care and support for all clients.
Our services can include, but are not limited to, the following tasks:
• basic housekeeping
• laundry
• meal preparation
• food shopping
• prescription pick-up
• bathing
• personal care
• escort service
Programs
We offer two client programs.
The Illinois Department on Aging Community Care Program – In this program, the state will pay for full or partial funding for eligible persons. They will send a Case Manager to the home to determine whether a person qualifies for the program. The Case Manager determines service hours and care plan.
JASC's Private Client Program – In this program, the JASC Home Support Services Supervisor will make a home visit to determine program eligibility, service hours and care plan.
The fee for home care is $15 per hour, with a 3-hour minimum.
Based upon homemaker availability, we can provide short-term assistance, such as the period following discharge from the hospital or emergency assistance. We serve private clients who live in the northeast Chicago area. For clients living further away, we may charge for additional transportation time.
Our client service scheduling for all programs is from Monday through Friday, with hours of service from 9 a.m. to 5 p.m. We do not provide service in the evening, on weekends, or on the holidays.
For more information about our programs and services, please contact the JASC by phone at 773.275.7212 ext. 228 or by e-mail.
