Adult Day Services is a program that offers supportive services to older adults and their families to enable the clients to remain independent and live at home. The program is structured to help the elderly maintain their independence and hold on to the quality of life and dignity they deserve. The program provides different activities to help maintain and develop physical and mental status to the fullest. As a service program, we are committed to:
   • assisting clients in their physical, mental and emotional needs;
   • providing clients with a stimulating environment, enabling them to enjoy their years and to share their vast experience with others;
   • providing caregivers with needed respite.

 
Program Overview
Location – the JASC, 4427 N. Clark Street, Chicago, IL 60640.
 
Hours of operation – Monday through Friday, 8 a.m. to 5 p.m. Closed on weekends and holidays.
 
Transportation is provided within the area bounded by Diversey and Peterson, and Kedzie and Lake Shore Drive. A wheelchair lift-equipped van is used to transport clients. The driver assists clients door to door.
 
Fee schedule
The private pay rate is $35 per day without transportation, and $45 per day with transportation (transportation is $5 one way).
 
Clients who qualify under the Illinois Department on Aging Community Care Program, or Veterans Administration guidelines, may attend either free of charge or for a nominal copayment fee.
 
Clients are billed only for the days they attend. Bills are mailed on the 10th of each month for the previous month's service, and are due upon receipt.
